Fellow Membership Requirements
Should be an Associate Member.
The applicant should be 1-year diplomate of the Philippine Board of Urology, Inc.
A duly accomplished Application Form. and pictures
A certified true copy of certificate of membership in good standing from the Philippine Medical Association.
Must have a photocopy of his/her current PRC license.
Sponsorship Letter from 2 Fellows in good standing addressed to Constantino Castillo, III, MD, FPUA, Chairman of the Credentials and Membership Committee.
Three (3) pcs 2×2 formal picture (white background)
Associate Membership Requirements
Must be a graduate of an Accredited Urology Training Program.
A formal letter of intent addressed to Constantino Castillo, III, MD, FPUA, Chairman of the Membership and Credentials Committee.
A Certified true copy of fellowship training abroad if applicable.
A Certified true copy of Medical School Diploma verified from original source.
A Certified true copy of Philippine Medical Board Certificate verified from original source
A Certified true copy of certificate of Residency training in Urology verified from original source
A Certified true copy of certificate of membership in good standing in the Philippine Medical Association
A photocopy of applicants current PRC license verified from original source
A sponsorship Letter from your Chairman and Training Officer addressed to MICHAEL JOHN FRANCIS V. GASTON, MD, FPUA, Chairman of the Membership and Credentials committee.
